The term "surplus" means the carrying out of occasional interventions which, in the opinion of the CO 118 competent for the territory, are of an urgent nature in the overall economy of the urgency / emergency system (for example: transport of the health team from / to the place of intervention, transport of the health team in order to resume operations, transport of technical personnel from / to the place of intervention, transport of personnel or material in the event of maxi emergencies, transport of blood, drugs, antidotes and organs, service emergency transport of patients to public hospitals and accredited health facilities, neonatal transport service etc ...)

In the decree of 15 February 2012 of the regional health department, it is indicated that the associations authorized to guarantee surpluses, as part of the Sues 118 system, must guarantee the departure of the vehicle within 120 seconds from the call. Demonstrating that what must count for those who are part of the volunteer network is the ability to offer an effective, quick service to the user. Precisely to regulate this aspect, the regional health department issued the decree “regulations relating to the use of voluntary organizations in the context of the surplus of the emergency-urgency service 118” published in the Official Gazette of the Sicilian Region n.10. The term surpluses is explained in article 1 of the decree: the carrying out of occasional interventions that, in the opinion of the 118 operations center competent for the territory, are of an urgent nature in the overall economy of the urgency / emergency system, such as the transport of the team health and technical personnel to or from the place of intervention, and also the transport of the health team in order to resume operations, or the transport of personnel or material in the event of major emergencies, transport of blood, drugs, antidotes and organs, as well as the emergency transport service for patients to public hospitals and accredited health facilities or the neonatal one. Within 60 days of the publication of the decree in the Gurs, the list of voluntary associations authorized to guarantee surpluses within the Sues 118 system must be established at Sues Scpa. certain requirements from associations. These must be registered in the regional general register of voluntary organizations, they must have at their disposal efficient rescue vehicles registered for the first time in less than seven years, with a distance of no more than 150,000 km. These vehicles must also be equipped with the prescribed health authorization verified annually by the competent ASP for the area. Another requirement is to have voluntary personnel with a certificate of suitability issued by the medical director of the association and every rescue vehicle must be equipped with a BLS D- PTC- PBLS D certificate and in any case must be in possession of the certificates and the relative operational capacity that will be previously verified by the 118 operations center competent for the area. Finally, the association must be part of an organizational network of regional or national voluntary associations, with presence in at least six provinces of the Region. This is to ensure continuity and professionalism through the coordination of the regional structure. Once registered, the voluntary associations will have the obligation to communicate their availability of the shift to the regional production center Crp of the Seus, indicating the coverage of the hours of availability, the license plate of the vehicle, the contact details of the mobile phone on the vehicle. and used for communications with the operations center responsible for the area. The 118 operations centers will ensure that the associations included in the list referred to in article 1 of the decree will supply the drugs deemed necessary and ready-to-use equipment “through the health districts of the ASP” which will be given on free loan.
